Care Management

Children aged 3 to 21, facing emotional challenges or chronic medical conditions, covered by or eligible for Medicaid insurance, may qualify for Care Management. This service assists in coordinating their medical care and ensures access to any other entitled services.



Here's how it works:

An evaluation will be conducted by a certified social worker to determine the child's eligibility for Care Management.

If the child qualifies, they will be assigned a dedicated care manager who will create a Plan of Care (POC) based on the child's needs, detailing the services they require and outlining the goals set for them.

The care manager will help the child enroll in all necessary programs, and communicate with the parents/caregiver twice a month to confirm the child's physical and emotional protocols are in place.

  • Transition Care
  • HCBS
